IF ALL INFORMATION IS PROPERLY SUBMITTED, WE CAN PROCESS YOUR APPLICATION QUICKLY.

EMERGENCIES ARE HANDLED ON A CASE BY CASE BASIS.

After submitting your application, please email copies of the following documents to:

​

            braziltownshiptrustee@gmail.com

or bring them to the office at 527 E. National Ave. If after hours, drop them through the slot in the door.​

​

  • Photo IDs for everyone over 18 in the household

​

  • Social Security cards for everyone in the household

​

  • Birth certificates for all children in the household under 18

​

  • All bills you pay on a monthly basis.  Including, but not limited to:

    •  Duke Energy

    • Brazil water,

    • CenterPoint gas

    • cable/wifi/internet

    • Cell phone

    • Other payments (Insurance, car payment, medicines, child support etc.)

​

  • A copy of your signed Lease (if renting)

​

  • For EACH adult member of the household  unable to work we need either:

    •  A Doctor’s note stating inability to work

                                     or

  •  A letter from Social Security stating the existence of physical or mental inability to work AND the amount of assistance they are providing

​

  • Proof of ALL sources of income for last 30 days:

    • Pay stubs, SSI, Child Support, TANF, Food Stamps, Etc.

​

  • Your latest bank or cash app statement(s) detailing all your deposits and expenses

​

  • Any other documents that may apply.
